Medical awards cover expenses incurred by the employee or immediate family members beyond those covered by medical insurance in cases of serious or life-threatening illness. Typical expenses that the foundation reimburses are airline travel for a family member accompanying the spouse or parent being treated off-island, hotel expenses, medical treatment co-pays, prescription co-pays and a per diem allowance. APPLICATION
Surgical Assistance Award
Oftentimes, an employee or immediate family member requires a surgery to address a non-life-threatening but disabling condition.  If you or a family member needs to have such a surgery, you may be eligible to receive up to $3,000 in assistance to help pay for medical treatment and prescription co-pays as well as travel necessary for the surgery.  Cosmetic surgeries or non-disabling conditions are not eligible. APPLICATION
Dental Surgery Assistance
An employee or immediate family member may require a dental surgery to address an urgent non-life-threatening, but disabling dental condition.  If you or a family member needs to have such a surgery, you may be eligible to receive up to $2,000 in assistance to help pay for dental assistance. APPLICATION
